If you wish to consent or have already consented to the COVID-19 vaccination for your child, but they have had COVID-19 in the last 12 weeks the NHS will not administer the Vaccine on Wednesday 24th November 2021. Full details on how they will check for this are in the letter.
Seasonal Flu and COVID-19 Vaccinations
Dear Parent/Carer,
The Leicestershire Community Immunisation Team are visiting our school on Wednesday 24th November 2021 to administer the seasonal Flu Vaccination and/or the COVID-19 vaccination (https://bit.ly/3DxOu6p).
They will administer both vaccines, just the Seasonal Flu vaccine or just the COVID-19 vaccine depending on what you consent to.
If you would like your child to receive either or both Vaccinations you must complete the online consent form by 11:00pm on Sunday 21st November 2021

It is important that if you wish your child to be vaccinated for either Seasonal Flu and/or COVID-19 that you read and complete these forms by the deadline. The NHS will not administer a vaccine to a student who they do not have consent for, and will be unable to accept consent on the day.
