Important information regarding data we hold about your child
On your child’s arrival at Countesthorpe Leysland Community College we receive contact information both from the primary school and from the admissions forms that parents complete. Occasionally there is conflicting information so we have posted home the information we hold for you to check and amend if necessary.
It is very important that the information we hold is accurate should we need to make contact.
Instructions:
- Check the information we hold is correct on the yellow data collection sheet (this has been sent in the post to your child's home address)
- Amend anything that is incorrect clearly in BLOCK CAPITALS.
- Note that the MyEd app (attendance, behaviour, achievements, school reports and how we send messages home) can only accessed by a priority 1 contact – please ensure this reflects who you want to be priority 1.
- Sign and date the form.
- Return to either your child’s form tutor OR the College reception by Friday 28th September 2018
May we also remind you to inform us of any changes as soon they occur (e.g. new phone numbers) so that we are always able to contact you in case of an emergency.
